About discharge/transfer support

When you are hospitalized, you need to think about what your life will be like after you leave the hospital at the same time as your treatment.
What kind of problems are there in order to live at home? What kind of medical and nursing care system is necessary? We will work together with you, your family, the attending physician, the nurses in the ward, and the hospital admission and discharge support station staff so that you can live a recuperative life with peace of mind.

In addition, in various situations, such as when it is better to continue rehabilitation, if you need to be transferred to another hospital instead of being discharged directly to your home, our support station staff will work with you, so please contact us.

About medical care support

We consult with outpatients and their families about the use of systems, treatment and nursing care methods, economic problems, and problems in daily life.We deal with a wide variety of medical concerns for patients and their families.
