An ordinary death is defined as an 'ordinary death' when a person falls ill and undergoes medical treatment and then dies from the diagnosed disease, while other deaths are referred to as 'unnatural death'.In other words, all deaths other than undiagnosed disease deaths, such as unexpected accidents, suicides, homicides, and unknown causes of death, fall under abnormal deaths.The person who discovers the unnatural death or the doctor who confirms the death will report it to the police, and the deceased will undergo an autopsy by the police.

The cause of death is determined based on external observations of the corpse, site conditions, and medical history.As a general rule, the corpse is not injured, but an examination (puncture test) may be performed to check for bleeding inside the body.Since the medical information obtained by external observation and puncture examination is limited, the accuracy of the cause of death diagnosis is not high.

XNUMX) Postmortem imaging examination

Imaging tests such as CT and MRI are performed on the corpse.It is possible to grasp the inside of the body to some extent without damaging the corpse.Examination time is 15 minutes for CT and 1 hour for MRI.Although the accuracy of cause-of-death diagnosis is higher than autopsy, there are many injuries and diseases that cannot be detected by imaging, and the cause-of-death confirmation rate is said to be around 30%.

XNUMX) Anatomy

A scalpel is inserted into the corpse to directly observe the inside of the body.At this time, organ fragments, blood, etc. are collected and stored, and after the autopsy is completed, microscopic examination, blood test, drug toxicity test, etc. are performed to determine the cause of death.It takes at least two to three months to complete all the tests.At present, it is the most accurate test method, but there are cases where no abnormalities are found even after exhaustive tests and the cause of death cannot be determined.

At our center, we perform the following three types of autopsies.

① Judicial autopsy

It is an autopsy that targets the corpse with an incident.The bereaved family cannot refuse an autopsy performed with the permission of the court.

② Consent dissection

Although there is no incident, autopsies are performed for corpses whose cause of death could not be determined by autopsy or postmortem examination.This will be done with the consent of the family.

③ Investigative autopsy

An autopsy performed in accordance with the Law for Background Investigation of the Cause of Death is called an investigative autopsy in Ibaraki Prefecture (there is no standardized abbreviation, and it is also called an autopsy by the investigative method, an autopsy by the new law, or an autopsy by the authority of the chief of the police station). .The autopsy is similar to consent autopsy in that it targets corpses for which the cause of death cannot be determined by autopsy or autopsy, but the consent of the bereaved family is not required, and the autopsy is performed under the authority of the chief of the police station.

If the death was due to an illness being treated, the cause of death will be determined without reporting to the police, as it does not fall under the category of unnatural death.
If the death is due to an illness other than illness, or if the cause of death is unknown, it will be considered as an unnatural death and will be reported to the police.

As a general rule, all cases are reported to the police.

XNUMX.Investigation of cause of death

The police investigate the deceased person's living conditions, personal relationships, circumstances leading up to the death, etc., and also check the exterior of the corpse (autopsy).After the police autopsy, the doctor will perform an autopsy.If necessary, postmortem imaging and autopsy will be performed to determine the cause of death.
In Ibaraki Prefecture, the implementation rate of postmortem imaging examination exceeds 50%, but the autopsy rate does not reach 10%.
