About admission permit (healing certificate)
The method for accepting admission permits will be changed as follows from December 12th (Monday).Thank you for your understanding and cooperation.
If you would like to receive a permit to attend school after contracting an infectious disease (certificate of cure), your child will need to be examined again.
If you wish to apply, please apply at reception counter ⑥ Medical Certificate Reception on the 1st floor after your re-examination.
(Monday to Friday, 8:30 to 17:00, excluding Saturdays, Sundays, and holidays)
Note 1) A document fee of 1,100 yen will be charged.
Note 2) Due to the time required for preparation, same-day delivery is not possible.
