   Insert your patient registration card into the ticket issuing machine and take a numbered ticket.

4. 

   Please put the file in the accounting slip.
   *Items to take home
   (Reservation slip, prescription, referral letter, etc.)

5. 

   The number will be displayed on the TV monitor.
   Please pay at the automatic payment machine or at the accounting counter.
   *If you cannot pay with the automatic payment machine, please come to the accounting counter.

About reissue of receipt

A receipt cannot be reissued.It costs 1,100 yen separately for a receipt certificate instead of a receipt.
Please apply at the accounting counter.It takes about XNUMX-XNUMX days from application to completion.
