This is a service that respects the opinions and requests of people who need nursing care and their families, and supports them so that they can live in their own homes with peace of mind.

A care manager with specialized knowledge in medical care, nursing care, and welfare will respond.

Creating a care plan

While consulting with the person and their family, we will advise you on the services you think you need, draft a care plan, and coordinate each service.

Who can use

In-home long-term care support is available to those who have been certified as requiring long-term care or support under long-term care insurance.

The diagram below shows the flow up to general use.However, you can use the service from the day you apply for long-term care insurance to the municipality.Please contact our office for details.

1. Apply to the municipality
2. receive an on-site survey
3. be certified*About 1 month from application to certification
4. Decide on a care manager in charge and start using the service
