Information regarding disclosure of records
In our home care business, we have formulated guidelines for the disclosure of records, etc. based on the "Guidelines for the Appropriate Handling of Personal Information by Medical and Nursing Care Businesses" (established December 24, 2004, revised April 21, 2006 and September 17, 2010) and the "Guidelines for the Provision of Medical Information (1st Edition: effective January 1, 2000, 2nd Edition: effective January 1, 2003)" and disclose records accordingly.
1. Records subject to disclosure
- (1) Records relating to users who have received services under this program will be collected for up to five years from the date of completion of the service.
- (2) Records, etc. at visiting nursing stations refer to the following:
Visiting nursing records, visiting nursing instructions, visiting nursing plans, visiting nursing reports, information provided to municipalities, etc. - (3) Records kept at home care support facilities include the following: home care support registers for each user, home service plans, records of assessment results, records of service provider meetings, etc., and records of monitoring results.
2.Persons subject to disclosure requests
- (1) In principle, the application (signing on the application form) must be made by the user himself/herself. In addition, if the user has passed away, only the legal heir or a person equivalent thereto may apply.
- (2) If you are a minor or if it is difficult to apply in person due to circumstances, please contact the relevant department.
3.How to apply
- (1) The person requesting disclosure must apply using the "Application Form for Disclosure of Records, etc." (prescribed by the organization), so please contact the relevant department to request the application form. When you contact us, we will also explain the necessary documents to attach, such as identity verification documents.
- (2) The person in charge will contact you when the issuance is ready.
- (3) Fees must be paid at the time of issuance.
4.When it is determined not to disclose the information
Records, etc. will not be disclosed in the following cases:
- (1) When the provision of the information or disclosure of records, etc. in question is likely to harm the interests of a third party.
- If providing information to the requester would likely make family members, service providers, or other third parties subject to attacks by the user. - (2) When the provision of information or disclosure of records, etc. is likely to seriously harm the physical or mental condition of the user.
- Patients with malignant tumors, mental illnesses, hereditary diseases, etc., who are deemed to have a psychological impact on the user and a negative impact on the effectiveness of treatment, even if the user is given a full explanation of the symptoms and treatment details. - (3) In addition to the preceding two paragraphs, when there are reasonable grounds that make it inappropriate to provide information or disclose records, etc.
5. Price (tax included)
- (1)Disclosure fee: 3,300 yen (per application)
- (2) Record copy: 33 yen/sheet (single-sided)
- (3) The cost of the administrator's supplementary explanation is 7,700 yen per session (approximately 1 hour).
