For those who purchase gift cards
I. (About using this gift card)
1. Gift cards can only be used at Tsukuba General Health Checkup Center.
2. Each gift card can be used by one person to receive the treatment course listed on the card.
3. As there is an expiration date, we recommend that you make a reservation as soon as possible after receiving the gift card. Reservations can be made by phone.
4. To use a gift card, you must make a reservation in advance. When making the reservation, please inform the doctor that you would like to use a gift card and provide the serial number on the card. Please note that if you do not inform the doctor that you would like to use a gift card when making the reservation and present the gift card on the day of your appointment, it will not be usable.
5. Gift cards can only be used by people residing in Japan who can understand, read and write Japanese and who promise to abide by these terms and conditions. For details on acceptance criteria, please see here.
6. If you wish to have optional tests, please apply when making your reservation. Fees for optional tests will be paid at the counter on the day of your appointment. For details on optional tests, please see here.

8. Please be sure to bring the gift card with you on the day of your appointment and present it at the reception desk. If you forget, you will not be able to be examined.
II. (Expiration date)
1. The expiration date of the gift card is one year from the date of issue. Please be sure to visit the clinic within the expiration date printed on the gift card.
2. Even if you change your appointment date due to illness or other reasons, please make sure to visit within the validity period.
3. Gift cards do not give priority to the reservation of the course printed on the card. If you do not make a reservation within the expiration date, the gift card itself will become invalid, so please make your reservation as soon as possible.
III. (Prohibited Items)
1. It is strictly prohibited to resell gift cards for profit, or to use them for commercial sales promotion or any other activity that may be deemed similar.
2. If any of the above actions are discovered, the gift card may be invalidated and you may be refused treatment. In this case, no refund will be given for the gift card fee.
Ⅳ．(Refunds, Losses, and Reissues)
1. No refunds will be made even if you are unable to take some of the tests due to poor health or illness.
2. Gift cards cannot be exchanged for cash or other cards, and cannot be refunded, repaid, or reissued.
3. We shall not be liable for any disadvantages incurred if the gift card is stolen, lost or destroyed, if the gift card is transferred, or if it cannot be used after the expiration date stated on the card has passed, unless such liability is due to intentional or gross negligence on the part of the Health Checkup Center.
