About applying for a receipt certificate

Receipts cannot be reissued. If you need to reissue a receipt, we will create a "receipt certificate" to prove that the medical examination fee has been paid. The creation fee is 1 yen (tax included) per receipt.

If you cannot come to the counter
①Please apply by phone
② Please send the application form and a copy of your identification document (or a letter of attorney + ID if you are acting as a proxy) to our medical examination center by post or email.
③After transferring the fee, please let us know by phone.
④Issued after payment is confirmed
⑤ By mail
